The 2nd Annual Spring Breakout Outdoor Soccer Tournament

WVU Men Club soccer would like to invite you to our Second Annual Spring Breakout outdoor Youth soccer Tournament. The tournament will take place at WVU Recreational Fields and Mylan Park Field. Schedule and Field address will be emailed after registrations are completed, and we look forward to hosting you and your team(s).

Brackets Available: U8-U18 Boys and Girl. We are going to offer a Gold, Silver and Bronze group for all teams.

Please send registration no later than February 20, 2012. Registration forms placed after the deadline will automatically be placed on the waiting list and late registration fee is $50 extra. Registrations without entry fee will NOT be accepted/considered without prior approval.

Each participating team will play a minimum of three games. These games (U8-U13) are scheduled to begin on Saturday morning. U14-High School games will start Sunday morning. If weather conditions dictate, the tournament committee reserves the right to shorten or cancel games, as necessary, without rebate of registration fees.

Entry Fee: $150 U8-U10, $175 U11- U12, $250 U13-U18(9v9), and $350 U13-U18(11v11)

Field Type: Artificial Turf and Grass

Tournament Format

� U8 � U10 � 6 - a-side including goalkeeper(6v6) 25min games

� U11 � 12 - 8- a-side including goalkeeper(8v8) 30min games

� U13�U18- 9-a-side including goalkeeper(9v9) 35min games

� U13�U18-11v11-a-side including goalkeeper(11v11) 20min halves

� Pool play plus finals

� Award for champion and runner-up

� No Max roster size but awards are limited to 15 per team

� Minimum of Three teams required for an age bracket. Teams in an unfilled bracket may play up one bracket or receive a refund. We reserve the right to combine brackets if needed. We reserve the rights to adjust length of games

I/we/parent/guardian realizes that by participating in this tournament, he/she may be injured. I/we/parent/guardian certifies that he/she is physically able to participate in this tournament.

MEDICAL RELEASE: I/we/parent/guardian hereby gives permission for any and all medical attention to be administered to him/her in the event of an accident, injury, sickness, etc. I/we/parent/guardian hereby assumes the responsibility for payment of any such treatment.

WAIVER: In consideration of his/her participation in this WVU Club soccer sponsored tournament I/we/parent/guardian do hereby for ourselves, heirs, executors, administrators, and assignees release and forever discharge the tournament sponsors, coaches, and all those persons involved in organizing and administering this tournament from all claims, demands, damages, actions, causes of action, or suits of action or suits of whatsoever kind or nature arising out of his/her participation in this tournament.

As coach I hereby swear that the birth dates and signatures listed above are legal and correct. As coach I will have verification of birth for each team member to present to tournament officials in the event that there is a question of age for any of my team members.
